DROOPS DAWGS RELEASE FORM
Droops Dawgs promises to respectfully offer only safe, sound and responsible care for my dog. However, I have been made aware of the risks involved with boarding my dog, including but not limited to interactions with other dogs and potential exposure to disease and parasites such as fleas and ticks. I hereby agree to hold Droops Dawgs harmless of any and all claims of injury, expense costs or damages caused by the actions of my dog while under their care.
I authorize medical care to be provided in the case of an emergency. Any veterinary costs will be reimbursed to Droops Dawgs. I agree to allow Droops Dawgs to administer or seek first aid and resuscitation where deemed necessary, I agree to hold Droops Dawgs harmless of any results thereof.
